The D.B.S. (Dynamic Brace System) is a patented system used for the treatment of idiopathic juvenile, adolescent, and adult scoliosis. It is designed as an attachment to the Vertetrac Lumbar Traction device.

Spinal curvature should not be diagnosed as scoliosis unless the curvature exceeds 10 degrees. Only curves greater than 10 degrees with structured changes including rotation should be diagnosed as idiopathic scoliosis. The curve progression should be considered only with a minimum increase of 5 degrees on two sequential X-rays.

Initially, the treatment with the D.B.S. is the same as with the Vertetrac.

D.B.S. Scoliosis Rail Indications
In patients with curvatures of less than 20 degrees, we apply the D.B.S. treatment only when the patient suffers from continued lower-back pain. The treatment is recommended for 30 minutes once a day until the elimination of the pain (about 15 to 20 treatments).
In patients with curvatures of 20 to 30 degrees, treatment varies depending on whether the patient is juvenile or adult. For juvenile patients who have no pain or other complaints, it is recommended to apply the D.B.S. treatment 2-3 times daily until maturity of the spine is reached. For these curves in children who do report pain, it is recommended to apply the D.B.S. treatment 3 times daily until the child reports no pain or until maturity of the spine is reached. For adults who report pain, it is recommended to apply the D.B.S. treatment once a day, and continue to receive treatments for 2-3 years to prevent regression. Adults who do not report pain but have a tendency for progression should receive the D.B.S. treatment for 30 minutes, 2-3 times daily until maturity is reached and the patient remains under control for 2-3 years.
In patients with curvatures of 30 to 40 degrees, D.B.S. treatment should begin immediately, 3 times daily for 30 minutes until maturity of the spine is reached. Patients should continue to receive treatment 1-2 times daily for an additional 6 months, and once a day for 2-3 additional years.
In patients with curvatures above 40 degrees, D.B.S. treatment should not be used. It is recommended to undergo surgery.
  In all cases, X-rays should be taken every 3 months to monitor the progress of the treatment. If progression of the curve greater than 5 degrees is discovered at any time, the patient should immediately begin to receive 3 daily treatments of 30 minutes each for 3 months. At the end of this time, another X-ray should be taken, and if progression of the curve greater than 5 degrees is discovered again, the administering medical professional should immediately re-evaluate the case.
 
For Achieving Optimal Results, The Strict Observation of the Following Indications is Absolutely Necessary:
1. One of the most important functions of the D.B.S. is the applying of distraction force. This must be done exactly as explained in the Vertetrac instruction Manual.
2. In C-scolioses, the applied distraction force should differential - much higher from the concave side. The spine should be straight during treatment. Pressure by the scoliotic rail pad should be applied on the convex side (on the apical vertebra). The force of pressure should be adapted to the endurance of the patient.
3. In S-scolioses (also double compound), the applied distraction force should be as strong as possible, but must be the same on both sides. Pressure by the scoliotic rail pad should be applied on the convex side, or on the hump when it exists.
4. In more complicated cases, when there is a doubt if the D.B.S.is properly applied, an X-ray should be taken (only during the first treatment). The improvement of the existing condition should be 60-80%.
5. In cases when the apical vertebra is at T-7 or below, the upper frame should reach the angulus of scapulae.
6. In cases with apical vertebra above T-9, the upper frame should reach the axillar fossa during traction (it is approximately at Th-4). In some cases the cervical traction should also be applied.
